Dear Parents/Guardians of Upcoming 7th Grade Students at Coffee County Middle School,

State law requires students in all grades in Tennessee public schools to be immunized against certain vaccine-preventable diseases.  In addition to vaccines your child has already received, state law requires one dose of the Tdap (Tetanus, Diphtheria and Pertussis) booster to be given PRIOR to your child enrolling in 7th grade for the 2021-22 school year. The full vaccine requirements for 7th grade students can be found on the TN Department of Health website at

We want to remind you to get your child’s vaccination early – before the busy back to school season begins.  Please contact the child's health care provider or the Coffee County Health Department (931-723-5134) to schedule an appointment for your child’s Tdap booster and an updated official Tennessee Immunization Certificate.  The updated immunization certificate must be submitted to Coffee County Middle School before your child begins the 2021-22 school year.  

There will be no grace/extension period for this required vaccination.  By law, if your 7th grader is not fully immunized for Tdap, he or she will be suspended from school until the required immunization is received.  If you feel your child has already received the mandatory Tdap booster required for 7th grade entry, please contact your health care provider or the Coffee County Health Department to obtain a copy of the updated immunization form to submit to the school.
